Where are my forms located?
Here are the directories where the forms are installed for each
version:

What's on the CD-ROM
As you may have noticed, your forms CD-ROM comes
with with some additional items this year. On the CD-ROM you'll find the following
items:

Where's the Subpoena?
In an effort to prevent fraudulent use of
the signatures on the subpoena, the subpoena is only available in
Adobe Acrobat read-only format. The form has been fielded so you
may enter information in the form if you have the Adobe Acrobat
v5 reader installed. The Adobe Acrobat reader is can be installed
from the main menu. For more information on installing Adobe Acrobat
Reader see Running the Installation.
You must also install the Adobe Acrobat version of the
forms as well. For more information on installing the Adobe Acrobat version
of the forms see Running the Installation.

General Information
Many of the forms has been redesigned this year.
We have made several improvements that should allow the forms to work without
spilling over onto extra pages without any problems. The forms were designed
to work with half-inch margins on every side. This should help those whose
printers have margins greater than a quarter of an inch. Because some of the
forms are so packed with information some forms may require some finesse in
order to fit them on one page. If you run into any problems with the forms still
spilling over onto other pages, please let us know.
All the forms where designed for an HP LaserJet 4000 printer.
They should work on any HP LaserJet or a compatible printer with at least a
1 megabyte of RAM. The forms were designed at 600dpi, but should also work
with 300dpi or higher. Since testing every single printer on the market is impossible,
we have tried to design the forms with some room for change. If you have any
problems with any specific printers, please let us know.

Installing Fonts
Unlike years past, this year font installation is not necessary.
Any fonts that you need will be installed automatically when you install the
forms.

Running
the Installation
Unlike years past, this year installation is very simple. The
CD-ROM is an autorun CD-ROM. When you place the CD-ROM into your CD-ROM drive,
after a few moments a menu will appear (unless you've turned off autorun). The
menu will guide you through the installation process and showcase some of the
other products that CyberDyne Industries has available.
If the menu does not come up (you've turned off autorun), you
can still manually run the installation menu.
- Insert the CD-ROM into the CD-ROM drive;
- After about 15 seconds (enough time for your drive
to read the information on the CD-ROM) if the menu has not popped up then
from your Windows desktop, double click on My Computer;
- Find your CD-ROM drive in the list of drives;
- Double click on your CD-ROM drive;
- Double click on CDSTART.EXE.
The only thing that you need to remember when installing the
forms is where you have the computer install them. Pay close attention so that
you'll know where to find them and use them.

Using the Forms
When underlining was used instead
of tables, a space was inserted at the beginning of the line to make it easier
to insert data. By moving the insertion point to the beginning of the line or
after the first space, you can type in any data that resides on that line. If
the underline overextends itself onto the next line, press the delete key to
remove the following tab and continue until the line is corrected.
To change a box in a form to an
“X”, first highlight the box using the mouse or keyboard and type the capital
letter “X”. A lower case “x” will likely put in some sort of asterisk. (Note:
For the forms with Easy-Fill Fields, the spacebar or mouse click will change
the box to an “X”)
If you make a mistake entering
information into the forms you can undo the last change that you made to the
forms from the edit menu in the Windows word processors.
Some of the forms do take savvy
when entering data. Use your common sense.
If you not sure where there are tabs and/or other non-printing
characters turn on the Show/Hide & button or turn it on from Tools…Options
(MS Word) or check the Reveal Codes (WordPerfect).

Forms With Easy-Fill
Fields
Easy-fill fields should provide you with an easy way to enter
data onto the Workers' Compensation forms. You should be able to open these
up, and enter your data without any problems. The "tab" key is used
to move in between the fields. "Tab" will move forward from field
to field and the "shift" tab will move backwards from field to field.

Using Easy-Fill Fields
Easy-Fill fields utilize Microsoft Word’s form feature. The
form defaults to being locked, but has fields that are unlocked for data input.
This enables you to enter data without modifying the underlying form.
The <Tab> and arrow keys can be used to navigate through
the forms. <Shift> + <Tab> can be used to navigate through the forms
backwards. If you need to use a <Tab> in one of the fields, press <CTRL>
+ <Tab>.
In fields where there is a check
box, pressing the <SPACE> bar will check the box. Clicking on the field
will also check the box.
You can also use the <ENTER> key in any Easy-Fill field.
For example, on Form WC-1 the hospital name and address are in one field. After
typing in the name, just hit the <ENTER> key to add a new line.
If you have a mouse it helps. You can still complete all of
the forms if you don’t have a mouse, but it’s the best $8 investment you can
make.

Unlocking the Easy-Fill
Forms
We have tried very hard to allow as much give as possible and
still comply with the State Board of Workers’ Compensation guidelines for the
forms. But as with most things in life, everything doesn’t always fit into a
nice little box; sometimes it can take a little finesse to get things to work.
If you can’t seem to get things to work and you need to change parts of the
underlying form, you’ll find it helpful to view the “Forms Toolbar”. It is under
View…Toolbars…Forms Toolbar in Microsoft Word.
From there you can unlock the underlying form and make any changes
you need. IMPORTANT: If you unlock the form after you have entered data into
the form, do not re-lock the form. If you do, you will lose any data that you
have already entered into the form fields. Save the form in the unlocked setting.

Basic Forms Information
To fill out the basic forms should be straightforward. Please
note that most of the information is filled out through the use of tables; however,
some fields use tabs. For these fields a space was provided before the underlining
begins. This should allow you to place the insertion point in after the space
but before the tab to type in the information. You can also delete the leading
space and just type in the information.

Color Paper
Please note that there is nothing on the form to indicate the
color of paper used to print out the form. Included with the disks is a list
of all the colors of the forms, number of pages that the forms should be, and
the form title. Please refer to this list when printing out the forms so you
know what color paper the form needs to be printed on.

Design Notes
The forms were designed with 0.4" margins. If you have
any problems with parts of pages not printing, please try shrinking the margins
back to 0.5" on all sides (Word:
File…Page Setup…Margins / WordPerfect:
Tools…Margins) and making the proper adjustments to the form. If the forms
are dumping over onto extra pages, try expanding the margins to 0.3" or
0.25" (Word: File…Page
Setup…Margins / WordPerfect: Tools…Margins).
If the page looks correct on the screen, but does not print
correctly, please make sure that the printer you are using has at least 1MB
of RAM.
All the forms were designed for an HP LaserJet or compatible

Wherever possible, tables were
used in the forms, allowing easier access when entering data into the forms.
A mouse is not required, but highly recommended when entering data into
the forms.

Updates
From time to time we will provide release updates to the forms
to make the forms easier to use and correct some problems that may occur with
formatting on specific systems. The updates will be provided for a small fee
to cover shipping, materials, and handling.
To encourage the use of e-mail, some updates may be available
at a reduced fee if e-mail updates are possible.

Any e-mail that you send should have “SCProbate” in the subject
header. Our e-mail system puts a priority flag on these messages.

No modifications to the appearance of the forms will be made.
Changes by the Probate Court are not considered
updates.
