South Carolina Workers' Compensation Commission Forms on Disc

The South Carolina Workers' Compensation Commission forms are provided on CD-ROM. Click here for a list of the forms on the CD-ROM. The forms include all the changes that have taken effect as of July 1, 2000. The included formats are:

• Microsoft Word: 6, 95, 97, 2000 & XP
• Microsoft Word with Easy-Fill Fields: 6, 95, 97, 2000 & XP
• WordPerfect: 6.x, 7.x, 8.x, 9.x/2000 & 10.x/2002
• CaseWizard: Word XP, Word 2000, Word 97 and WordPerfect 9.x/2000

Licensing: The CD-ROM can only be installed on a single workstation - not a network server. You must purchase a license for each workstation on which the CD-ROM will installed. If you would like to install the CD-ROM on a network then you must purchase a network site license. Under no circumstances can you install the forms onto an Internet server.
